About 40 percent of the practices surveyed by The Health Care Group Inc. lost a receptionist last year — the biggest burnout job at the office. But that's just the beginning: 9.5 percent lost a medical assistant that year, 6.3 percent lost a clerical worker, and 4.4 percent a registered nurse.. That kind of turnover is costly. Non-physician salaries, the single biggest drain on practice income, account for more than a third of physician expenses. Each time you replace an employee, the burden grows heavier. In fact, the rule of thumb is you'll dish out an estimated 1.5 times the salary to refill that position — in other words, $30,000 to replace a $20,000 employee.. The reason: You'll pay to advertise the position, for the time to interview and hire, for training, for lost productivity and perhaps for overtime.. "Most of the costs are hidden," says C. David Carpenter, a principle with PCSi HealthCare Consultants in Southern Pines, N.C. "People misunderstand the value of a long-term employee." . Employees leave for a variety of reasons.
