Abnormalities of microvascular responsiveness to neurohumoral agents probably contribute to the etiology of many diabetic complications. Evidence has accumulated showing that DM influences vascular reactivity to drugs. Though noradrenaline and tyramine were equally effective in normal and diabetic animals, the responses to histamine, bradykinin and platelet activating factor (PAF) are reduced in diabetic in comparison with non diabetic rats. Insulin either systemical or topically administered restores the reduced responses observed in diabetic rats, indicating that the defect was related to the diabetic state (Fortes et al., 1989,Gen. Pharmac. 20: 615). Alterations in the renin-angiotensin system (RAS) such as decreased in renal and plasma renin activity, enhanced reactivity to angiotensin II have been demonstrated in DM. Clinical, experimental and molecular biologic studies all invoke an important role for RAS in the pathogenesis of diabetic complications, mainly in diabetic nephropathy (Jaffa et al., 1997, Diabetes 46:2049).
